How do I apply for or renew licensure online?
Apply online at www.BreEZe.ca.gov.

What do I do if I cannot move any further on the Renewal Survey question?
Use the following format (numbers only)
- Place of Practice: 5-digit Zip Code – XXXXX (ex. 11122)
- Hours of Practice: Number of hours – XX (ex. 40)
If you do not have a place of practice (i.e. retired, not employed), use your home zip-code. In the hours of practice enter 0.
Why isn't my RDH/RDHAP/RDHEF license on the Quick Start Menu when I log into the BreEZe system?
The license type must be added to your BreEZe profile. Click on the Update Profile button located in the upper right-hand corner of the Quick Start menu and add your license. Be sure to follow the example provided when entering in your response:
- Last 4 of SSN = XXXX (ex. 1111)
- DOB = MM/DD/YYYY (ex. 01/01/1999)
Why am I getting an error that says, “Entity already exists”?
This error means you have an existing account with BreEZe that you previously created. You receive this error when attempting to create an account with a different email address. BreEZe allows one email address per individual. Please contact the DHBC by completing the contact form or by calling (916) 263-1978 for further assistance.
Why am I getting an error that says, “No records match”?
There may be an issue with your Name, Date of Birth (DOB) or SSN.
- Common Name errors – Spacing between names. Names must be entered exactly as they are in BreEZe. Ex.
- McDaniels (No space between Mc and Daniels)
- Mc Daniels (Space between Mc and Daniels)
- Last 4 of SSN = XXXX (ex. 1111)
- DOB = MM/DD/YYYY (ex. 01/01/1999)

I paid my application and/or renewal fees and the system errored/timed out, what now?
The BreEZe system may be temporarily down. Generally, when this occurs your payment did not go through. You will not be charged twice.
Log back into your BreEZe account, click on Make Payment Cart (1), which should to the right of your screen under additional activities. It should take you straight to the credit card payment section.
Complete the credit card information and submit the payment.
